The initiative aims towards spreading awareness about legality of this important identity document which must be received by the parents for future security of a new born. To check NDMC Birth Certificate application status online, visit MCD website http://www.mcdonline.gov.in/ and choose your region (NDMC, SDMC, EDMC). Then select Application Status Section and enter reference number of the application. All the details related to birth registration application will appear. One can also visit different Delhi Birth registration centers of the region. There are more than 180 centers of MCD including 50 rural and 135 urban centers that are available to address common queries and registrations. However, certificate is only issued by the deputy health officer or designated registrar of birth of particular Municipal Zone.

Documents Required for MCD Birth Registration Application

When an applicant submits Birth form with documents, a draft order is made and sent by the concerned authority (region’s registrar) to nearby police station for verification. After positive verification report, an order for issuing MCD birth certificate is granted. To receive birth certification on time, download the MCD birth application form and fill accurate details. Scan the documents as given below and submit it online with application fee and form. One can also submit hand-written application in the prescribed format with fee and an affidavit, if going for offline mode. Here is the list of documents required to make Delhi birth certificate.

  • Completed application form. One can download the form from official website or can collect the form from nearby registrar office.
  • Affidavit in prescribed Performa specifying place, date and time of birth signed by authorized officer.
  • Supporting document for the present residential proof like Voter ID, Electricity bill/water bill/house tax on parent’s name/Ration Card for address proof.
  • Document of the address child occurrence took place.
  • Copy of Ration Card/Aadhaar card/Voter Id card/Passport of parents.
  • Copies of Parent’s identity and residential documents of Parent’s attested by a Gazetted officer.
  • AADHAR card Enrolment No. and any one of the identity proof PAN / PASSPORT / Driving Licence / Voters Card / Identity Card. The name of the applicant on the AADHAR card should match with the Name of the Applicant.
  • A letter written by Medical Officer Incharge, any officer authorized person of Hospital/Health Centre, Maternity Home where the birth has occurred.

Go to the Check Status, Certificate Search or Downloads section as per your requirement and enter reference number. A new page or tab will appear with further details. Procedure to Change Name in Delhi Birth Certificate

Name change in birth certificate is one of the difficult tasks if the registration has been done priorly. Due to the legitimacy of the document, there is a time period prescribed to make changes in name of birth certification. That is only within 1 year of birth, name change in birth certificate application can be submitted. After that, for no reasons, applications will not be accepted at any regional centers.

For instance, if name change is to be done within 1 year of birth then it can be done via Citizen Service Bureau through online mode. One can also submit an application form with affidavit mentioning the reason for change and attested by Gazetted officer, to be submitted at regional centers.

Changes in the certificate can be undertaken as written request along with an affidavit as well by concerned authoritative person at any of Delhi registrar office. There should be complete application form, supporting documents and strong reason behind such change, otherwise application can be rejected by the department in-charge.

Correction in Birth Certificate

The process to make changes in Birth registration is quite simple as registering online for new certificate. One can either to it online or can visit nearby Municipal registrar office as per the regional area. To do it offline, a correction letter in prescribed format is required from the Medical Record Officer of concerned hospital or birth center where the event occurred along with supporting documents for events. With Birth certificate correction fee of Rs.10/- duly attested by SDM/Notary public, Affidavit stamp paper mentioning reason and subject correction of birth events must be submitted.

Other identity documents of parents such as Passport, Ration Card, Voter Identity Card, 10th class certificate, receipt of certificate is required. For online procedure, visit website of respective Municipal Corporation and submit correction form with accurate details and fee. Get the online receipt and wait for verification and further follow up from the department. After completion of the process, to get birth certificate it takes a week to receive the document at the residential address of the applicant.

Birth Certificate Fee

There is very nominal fee for birth registration imposed with first copy of birth application from the parents within 21 days. Only Rs 7/- registration fee is applicable along with form within 21 to 30 days of the new born occurrence.

If the period still extends up to 1 years then late fee charges will be Rs 10/- along with SDM Order from local area. Additional Rs. 10/- will be charged to register name and Rs. 20/- will be charged for one copy. For more copies Rs 5/- is charges extra. For searching any old birth record, Rs. 7/- will be charged as a fee that must be submitted offline at respective department.
